Bipolar disorders are a type of mood disorder. There are several subtypes of bipolar disorders. The most common ones are Bipolar 1 and 2 disorders. These affect approximately 5 percent of the population, which means that we have about 1.5 million people with this condition in Canada. Bipolar disorder is a serious psychiatric condition.

We all forget things as we age, but that doesn’t mean we have dementia. Dementia isn’t just one condition - it’s a term that refers to symptoms that cause a decline in mental ability that interferes with daily life. Alzheimer's is the most common type of dementia and accounts for 60 to 80 percent of cases. Vascular dementia is the second most common dementia type and occurs after a stroke.   • Generalized Anxiety Disorder Diagnosis and Treatment " Erin a 34-year-old married mother of two young children"

    Erin is a 34-year-old married mother of two young children, and works as a store manager. She presents to her family doctor complaining that it is becoming increasingly difficult for her to function at work because she is constantly having trouble concentrating, and is also feeling exhausted all the time because she sleeps poorly at night. On further questioning, she reports that for the past year or so since her eldest child started going to school, she has been feeling very anxious about how he is doing in school, including whether she packed the right lunch for him, whether he has the right school materials, how he is interacting with the other kids, etc. This worrying has also spread to other parts of her life, and she has been fretting about her family’s finances, about the right foods to buy for her family, and also about issues at work. Her husband has been telling her that she worries way too much, and she acknowledges that there is no good reason why she should be worrying so much, but yet can’t seem to control these worries. These constant ruminations are what keep her up at night and prevent her from sleeping. She has also been feeling on-edge, irritable with her husband and children, and having a lot of stomach aches and loose stools.

    Treatment: This patient is diagnosed with Generalized Anxiety Disorder. She is referred to a psychologist for Cognitive Behavioural Therapy (CBT).

  • Bipolar II Disorder Diagnosis and Treatment " Frank a 55-year-old advertising executive "

    Frank, a 55-year-old advertising executive, has started seeing his new family doctor, and reports that he has “always been depressed” for as long as he can remember, and certainly since adolescence. At different times in his life his depressions could be pronounced, and for a couple of weeks he would be isolating himself in his home, cutting off contact with family, friends and colleagues, and feeling suicidal. These episodes would usually resolve spontaneously, and he never went for treatment, except once when he was prescribed an antidepressant for a short time, but stopped it when it made him feel “more depressed and irritable”. Much of the rest of the time, his moods would range from “down and lonely” to anxious, and also highly irritable. In fact, it has been his tendency to feel irritable and to have a volatile temper that has characterized his personality, he feels, and the people who know him tend to see him as someone who can be at times passionate, driven, yet also explosive and unpredictable.

    When asked about his career, he explains that he is a very successful businessman, who managed to grow a very prominent advertising firm. He describes having a capacity to get fully involved in work projects where for weeks on end he will barely get any sleep, feel full of energy, highly motivated by big business projects, highly impatient and irritable when feeling that others are not keeping up with him, racing thoughts, rapid speech, and also high libido at those times. On some of these occasions he would take very big financial risks, spend very large amounts of money on his projects but also on lavish parties, gifts for colleagues and business partners, though he never saw anything wrong in this.

    This patient was referred to a psychiatrist for a consultation, and diagnosed with Bipolar II Disorder based on his reports of hypomanic episodes and depressive episodes throughout much of his life. He was found to be currently in a Major Depressive Episode. Rapid cycling was also mentioned in the diagnosis, given that he seemed to have multiple discrete mood episodes in the same year. He was prescribed Seroquel XR 300mg qpm, as well as Metformin 500mg BID to prevent possible weight gain and metabolic syndrome from the Seroquel.
