Many people coming in seeking advice on age-related changes that they are seeing on their faces need to understand that these changes are occurring because of a combination of factors.
Dynamic lines can be etched into the forehead or crow’s feet from continual muscle movement. Volume loss occurs in the cheek, nasolabial fold and marionette lines. Textual changes occur as well as pigment changes in the skin and you have some sagging due to gravitational changes.
Dynamic lines can be addressed with neuromodulators, such as Botox, Xeomen or Dysport. These proteins bind to receptors in the muscles and prevent them from responding to the neural impulses that make them contract.
This allows the overlying skin to relax, gives it a break and reduces the appearance of these lines. Volume loss that occurs with aging such as in the cheeks, nasolabial folds and marionette lines is very often addressed with hyaluronic acid-based gels. These fillers are injected using needles or cannulas and can provide an immediate result.
Pigment changes and textural changes in the skin are often neglected when we look at using combination treatments, with fillers or Botox. It’s important to understand that in a patient that has retained volume or beautiful, unlined skin; typically they’re at an age where their skin has retained its radiance and even skin tone.
So when we’re addressing the patient’s issues with a combination approach with Botox and fillers, it’s important not to forget the canvas that we’re working with. Gravitational changes that occur with aging are often addressed using surgical interventions, but not all people feel comfortable going under the knife or wish to choose invasive procedures.
Some non-invasive modalities that are available include radio frequency and ultrasound modalities. These treatment modalities deliver energy deep into the skin. They stimulate collagen and allow for some tightening to occur.
The impression of increasing age and fatigue that concerns many people is really caused by a combination of factors and any treatment strategy should take all these factors into consideration.
