The London Family Health Team offers high quality primary health care for its patients in London Ontario. An interprofessional group of health care practitioners, including family physicians, provide health care in an integrated, respectful and holistic environment. The focus of the family health team is on chronic disease management and prevention as well as health promotion. By working collaboratively, the team is able to provide a broad spectrum of primary health care services to its patients to ensure they get the care they need when they need it. Family Health Teams are an important part of changing both the health care system and the way people think about health care. Family Health Teams focus more on preventing illness and promoting health. The teams look at the best ways of managing chronic illnesses such as diabetes and depression. In doing so they are able to provide care that is more accessible and comprehensive and gives patients the opportunity to be more involved in making decisions about their own health.
