Working with the client and the family, we then develop and monitor a care plan that evolves as levels of care change.
Équinoxe Care Managers are skilled nurses or social workers, with recognized credentials in gerontology, home care and nursing. Their expertise enables them to structure the most appropriate support for short or long-term care. Families can feel secure that their loved ones' interests are completely protected. Équinoxe Care Managers act as both consultants and advocates. Maintaining client confidentiality is of the utmost importance.

Équinoxe Cancer Care is a multidimensional service for clients undergoing all phases of cancer treatment. Our teams provide care during in-hospital procedures and follow up assistance at home after treatment.
