Care Management is the coordination and supervision of a wide range of in-home services to maximize the well-being and comfort of clients. Our care managers begin with a thorough assessment of each client’s physical, social and emotional needs. We plan for meeting nutrition needs, safety and cognitive functioning as well as health care and medical requirements. Working with the client and the family, we then develop and monitor a care plan that evolves as levels of care change.
Équinoxe Care Managers are skilled nurses or social workers, with recognized credentials in gerontology, home care and nursing. Their expertise enables them to structure the most appropriate support for short or long-term care. Families can feel secure that their loved ones' interests are completely protected. Équinoxe Care Managers act as both consultants and advocates. Maintaining client confidentiality is of the utmost importance.
Équinoxe is a member of the National Association of Professional Geriatric Care Managers (NAPGCM) and our caregivers follow the association’s standards of practice.
Cancer Care
Équinoxe Cancer Care is a multidimensional service for clients undergoing all phases of cancer treatment. Our teams provide care during in-hospital procedures and follow up assistance at home after treatment. This can include emotional support, help with personal care and accompaniment to medical appointments.
Senior Care
The tasks of daily living often become unmanageable for the frail or disabled elderly. As a trustworthy and affordable alternative to residential or nursing home care, Équinoxe LifeCare offers professional support for medical, physical, emotional and social requirements for those suffering from Alzheimer’s and other diseases of aging. Supervised by a professional Care Manager, the Équinoxe team can offer the highest standards of assistance with all aspects of personal care.

Working as part of a multidisciplinary team led by Palliative Care specialists, our highly trained teams of Registered Nurses, Registered Nursing Assistants and nurses’ aides provide occasional or 24-hour professional support and respite care for clients and their families. The key element of this quality care is pain and symptom control for the client and emotional support and respite for the family.
